Caching Rework: New SQLite Structure

Implement new SQLite structure for persistent data structure as described in the [Caching RFC](https://github.com/apollographql/apollo-ios-dev/blob/design/rfc-caching/apollo-ios/Design/rfc-caching-rewrite.md#sqlite-structure).
