manifest.yml のenv の設定がそのままでは動きませんでしたので修正しました（行先頭に空白を追加）
node の cloudant パッケージが、バージョンによってはうまくコンパイルできずPushが失敗することがあるようなので、バージョンを固定しました。
READMEで一部説明不足の部分を追加しました。
ご検討いただき、マージお願いします。
