Unverified role sometimes not added to new members

I manage a discord server where we use emailbot to verify our members.

We have our unverified role set to disable all channels. If a user doesn't have this role, they are able to use the server as normal. We have to do this as discord onboarding requires the @everyone to be able to see a certain number of channels, but we want to limit that, so we need the unverified role as a "blacklist" on top.

Recently, I've noticed some new joiners are not being granted this role upon joining. I set up dyno to add the role to new joiners as a backup. Not sure if it's a bug or just an uptime issue, but thought it was worth flagging.

I've found some even stranger behaviour now. With both emailbot and dyno adding the role, they still occasionally do not recieve the role despite audit log claiming both bots successfully added the role! I think there is some sort of issue occurring on discord's backend to do with onboarding as members are not recieveing the unverified role and the audit log does not agree!
