When enabling a course via LTI, create a TeacherEnrollment type record for the user and course. Without this record an unauthorized response would be returned, and on the UI side we would have to know that in that specific case, unautorhized didn't mean unautorized, rather that the data is not yet loaded for the course. Now we can handle that cleanly.
